An unplanned event or circumstance, that could have or did lead to harm. It can include any event that may have given rise to physical, emotional or psychological harm. Incidents also include:
- the unauthorised use of restrictive practices,
- any workplace hazards with the potential to cause injury or property damage, and
- any near misses which had the potential to cause harm but did not on this occasion.

We all have a responsibility to prevent, respond to, manage and report incidents, hazards and near misses that occur when providing supports or services to people. Timely and effective responses to reportable incidents improve the safety and wellbeing of the people we support, ourselves, our colleagues and the community. Where a significant incident has occurred, an internal review will be conducted to identify any areas for improvement and enable future risks to be managed. |
